Referral Process

Referrals for our Permanency Services program, Day Treatment and Residential services can be made to the Compliance Director, Shanon Staires at, 970-344-1380 x220, or 970-556-3982. Supporting documentation that may be helpful in determining appropriateness for placement in one of our programs include, but are not limited to:
1. Current IEP
2. Psychological/Neuropsychological or Psychosexual Evaluations
3. Family Services Plans
4. Treatment summaries from past providers
5. Court or probation documents
This information can simply be emailed to or can be faxed to 970-344-1394.

A phone conversation about presenting issues and reasons for referral is always helpful.  The information is then provided to the relevant department heads for review. Consultation takes place with RFY teams for final intake approval. If concerns are still present after consultation, Reflections may choose to schedule an interview with the youth and/or family. Assessments may be completed if the client is a private insurance or Medicaid client. After these initial steps, an intake can be scheduled and completed.

Reflections for Youth now accepts Colorado Access insurance in addition to Medicaid and some private insurance plans, and is a Value Options provider!
